- Better Health for the Delta
The Southeast Louisiana AHEC grantee for the Louisiana Mississippi Delta initiative combined our activities during Phase I and Phase II with the Robert Wood Johnson RWJ) Southern Rural Access Grant Program (SRAG) rural loan fund component and created "Better Health For the Delta".

Healthy Communities of Louisiana-The Obesity Project is a health education and screening project targeting obesity and related diseases such as diabetes, coronary heart disease, and stroke in at-risk African American adolescents and adults.
- Louisiana Learning About Pregnancy and STDs
This project was designed to address the health care status of rural teenagers in three poor, rural parishes by providing peer-based health education, recruiting and training peer health educators in each parish, and conducting presentations on STD and teen pregnancy prevention to local civic groups, churches, and schools.
- Maternal Infant Health Outreach Worker Program
The Vanderbilt Center for Health Services’ Maternal Infant Health Outreach Worker (MIHOW) program provides early intervention services to low income families in Nashville as well as other urban and rural areas in Appalachia and the Mississippi delta, including Arkansas, Kentucky, Louisiana, Mississippi, Tennessee, and West Virginia.

- Northeast Missouri Rural Health Network Prescription Drug Assistance Program
This program assists individuals by gathering the needed data and completing the necessary enrollment forms for free medications available through pharmaceutical companies, generic prescriptions through RxOutreach and/or TogetherRx drug discount cards depending on their income level.

Dedicated teachers, and a "sense of community" figures prominently in the academic success of the rural Fruithurst elementary school.
- Shared Information Technology (IT) Services
The Community Health Network was formed to share information technology (IT) resources among varying sized community health organizations across Tennessee, from one doctor, one location, to much larger groups in multiple locations.
- South Cameron Memorial Hospital Rural Health Outreach Grant - Telemedicine Project
The outreach project has established seven Telemed sites throughout southwest Louisiana - five in rural hospitals, one in a facility for the developmentally disabled, and one in a mobile telemedicine clinic (a 36-foot Airstream recreational vehicle that travels to rural sites for health screenings and telemedicine encounters).
