- Better Health for the Delta
The Southeast Louisiana AHEC grantee for the Louisiana Mississippi Delta initiative combined our activities during Phase I and Phase II with the Robert Wood Johnson RWJ) Southern Rural Access Grant Program (SRAG) rural loan fund component and created "Better Health For the Delta".
- Chronic Care Education Outreach Program
The consortium for the Chronic Care Education Outreach Program will expand an existing chronic illness self-management education program to focus on the elderly in Woodruff and Prairie counties in the Arkansas Delta region.
- Delta Community Partners in Care
Delta Community Partners in Care (DCPIC) is a coalition of partners serving a rural area in the Mississippi Delta region of northwest Mississippi. Its target population is the uninsured or underinsured who have a diagnosis of diabetes, hypertension, or both.
- Dubach Health Outreach Project
The purpose of this health outreach project was to access a multidisciplinary community-based
intervention to combat obesity and related chronic diseases in Dubach and the neighboring communities.

Healthy Communities of Louisiana-The Obesity Project is a health education and screening project targeting obesity and related diseases such as diabetes, coronary heart disease, and stroke in at-risk African American adolescents and adults in the parishes of Bienville, Lincoln and Jackson.
- IT Project Connects Rural Louisiana Hospitals
Describes how Louisiana Rural Health Information Exchange (LARHIX), an information technology project bringing together 24 rural hospitals and the Louisiana State University Health Sciences Center at Shreveport (LSUHSC-S), works to fill the gaps in patient record sharing among rural Louisiana hospitals.
- Maternal Infant Health Outreach Worker Program
The Vanderbilt Center for Health Services’ Maternal Infant Health Outreach Worker (MIHOW) program provides early intervention services to low income families in Nashville as well as other urban and rural areas in Appalachia and the Mississippi delta, including Arkansas, Kentucky, Louisiana, Mississippi, Tennessee, and West Virginia.

Examined three community transportation models for non-emergency medical services.
- Northeast Missouri Rural Health Network Prescription Drug Assistance Program
This program assists individuals by gathering the needed data and completing the necessary enrollment forms for free medications available through pharmaceutical companies, generic prescriptions through RxOutreach and/or TogetherRx drug discount cards depending on their income level.
- Rural Alabama School Sets High Standards
Dedicated teachers, and a "sense of community" figures prominently in the academic success of the rural Fruithurst elementary school.
